Please help get me started

how would you start a letter of reference request?I have had a professor 5 years ago while at community college. In the past I have asked him in person to write me a letter of recommendation for professional school. Now I have my bachelors degree and want to move on and get my M.A in my field. I dont know how to ask him via email to write on my behalf to masters of arts program. I think I just need some examples of just how to start a sentence or two on how to ask."I was wondering if you could write on my behalf to the masters of arts program"How would you ask?ALSO i wanted to include that out of all professors i feel that he is the professors that knows where my academic strengths are BUT wirting is not my strongest area, I dont know how to make that sound more profession. PLEASE HELP ME GET STARTED!
Answer1:you posted this under small business...what kind of degree do you have?
Answer2:shortest route to any place, a straight line
Answer3:I think you should call him - its more much more courteous, you dont have to worry about how to phrase a letter, and you can get immediate feedback from him about his willingness to assist you. You are asking him to make an effort on your behalf, so you should be willing to make the effort to actually talk to him, or at least leave him a message that you would like him to get in touch with you.
