How should you write a buisness letter

I really want to write a letter to my psychiatrist discussing myself and confidential details about me, i also want to forward this letter to my GP, I want the letter to be informative, and to sound clever. Shall I put lots of long clever words in so i seem smart? Can I used connectives like futhermore,thus, in addition, or shouldnt i do that. Is a buisness letter informal or formal, help!
